Extramedullary hematopoietic effusion (EHE) is one of the extremely rare phenomena associated with extramedullary hematopoiesis, which caused by serous effusions, including pleural effusion, and may be related to hematologic disorders neoplasms. Herein, we present case an 81-year-old man EHE accompanying Waldenström's macroglobulinemia (WM). The patient complained anemia dyspnea. chest X-ray computed tomography showed a massive left aspirates revealed infiltration immature myeloid cells megakaryocytes, in addition lymphoma cells.
